To fully upgrade your Leviathan Axe, you will need to find eight Frozen Flames. Most of the Frozen Flames are story-bound and you will be able to get them by just playing the main story of the game however some of the Frozen Flames are locked behind special tasks that you will need to complete before obtaining the few of the remaining ones. In addition, if you forget to pick any one of these Frozen Flames, you will find them sitting inside the blue chest at any of the blacksmiths.

Defeat The Huntress boss and you will get your very first Frozen Flame.
Frozen Flame #2
Defeat the Alva boss and you will get the next Frozen Flame.

Defeat the Nidhogg boss and you will get the next Frozen Flame.
