Mysterious Items can be easily missed if you cannot collect one. But the game does give you a chance to play each Act again and separately to collect any Mysterious Item you may have missed. This can be done by finishing the game, which will then unlock New Game + where you collectibles will be carried over.
It is worth mentioning that any Mysterious Item you collect will not be saved until you reach a new checkpoint. This means that if your character dies before you can reach the next point, every collectible will be lost and have to be picked up again. Another thing to keep in mind is that your collectibles tracker to unlock the trophy is only updated when you get back to the cabin. This is why you should finish the Act you are playing through.
If you miss a collectible and use New Game +, you can go to each Act to play it again and collect any Mysterious Item that you forgot to collect. Make sure that each time you play a different Act, you will have to finish it and then return to your cabin for the trophy to update its tracker.

Mysterious Item #1
When you are locked in one of the steam rooms to avoid the monster, you need to wait for the monster and then leave to exit back out. To progress the story, you will need to go right with the projector, but for this item, you need to go straight ahead.
In this new area, you need to press the switch and then rush back to crouch under the pipes to get past the finish line before the time is up. Here you will find the item on a table.
Act 3 – Bloody Roots

To get this item, you need to solve the puzzle in the tree truck after you reach the garden area. The collectible will be on a child’s lap on a bench.
